The Role of In Home and Community Care Planning Services in Supporting Carers of Older Adults

Care Plan Managers: Our Care Plan Managers (CPM) focus on creating a cost-effective care plan with a focus on coordinating service provision and improving communication between healthcare providers, the older person and their family.

Examples of the role of the CPM include: scheduling and attending appointments, providing regular updates to family members, connecting the person to community services and coordinating care with all team members.

In-home services: We provide in-home care plan management and services including:

  • Chronic disease management
    • Home and community safety assessment
    • Occupational Therapy functional assessment to identify solutions for difficulties performing self care tasks
    • Mobility assessment to determine proper and safe mobility devices and to assist with government funding application
    • Home accessibility assessment to recommend modifications needed due to functional limitations and reduced mobility
    • Social worker support for carers and for older adults.
    • Rehab Therapist to help maintain daily routine, social engagement, reduce social isolation and carry out cognitive stimulation programs

These services are designed to support the independence of older adults, while offering respite and support for carers.

Dementia care: We provide strategies to manage and reduce responsive behaviours and interventions to support the independence of the person, as their condition progresses.

All our services are focused on supporting the wellbeing of older adults, by maintaining and enhancing safety, autonomy, daily routines, and participation in activities that they enjoy.
